 This form is to be completed each month and mailed or emailed to the District Governor, Vice-District Governor, Region Chairperson, and Zone Chairperson. 

Note:  The form is in Rich Text Format and should be read by most text editors, i.e... WORD, WordPerfect, Works, etc...  save this form in your "My Documents" folder or a folder where you can find it each month.  When you update it, be sure and save it under another name that will identify it for the month of update.  If you experience any problems please e-mail me and let me know what they are.

After the form is filled in, you can then e-mail it to the respective cabinet members by attaching it to your e-mail.  It might be nice to let each of them know that there is an attachment.
